APPLYING FOR NEW SERVICE CONNECTION

  • ABOUT THE SERVICE:
    • The connection will not be made until the application is approved and all charges were settled.

  • WHO MAY AVAIL THE SERVICE?
    • All bonafide residents of Jaen, Nueva Ecija

  • SCHEDULE OF AVAILABILITY OF THE SERVICE:
    • Monday to Friday
    • 8:00 am – 5:00 pm
    • No noon break

  • WHAT ARE THE REQUIREMENTS?
    1.) Accomplished Service Application form
    2.) 1 pc. 2×2 picture
    3.) Photocopy of valid ID (company ID, drivers license, Postal ID or any valid ID acceptable to JWD)
    4.) Brgy. Clearance
    5.) Authorization letter from lot /building owner (if applicant is a tenant)

    * Any other proof of residency acceptable to JWD

    Additional requirements for commercial class:
    • Photocopy of Business / Mayor’s / DTI permit

  • DURATION (under normal circumstances):
    • Application within 10 minutes depending on the complexity
    • Inspection within 2 working days
    • Service Line Tapping 5 – 10 days

  • *The Management reserves the right to reject any application if not passed with the required criterion.

    *All ID’s should bear signature, picture and exact address.

HOW TO AVAIL THE SERVICE

  • STEP 1

  • CLIENT :
    • Present applicant’s copy of inspection slip and fill-out SACO and submit photocopy of valid ID , 1 pc 2×2 picture, Barangay Clearance, Certificate and authorization if necessary

  • SERVICE PROVIDER:
    • Prepare concessionaire SACO/Contract form and action report.
    • Advice the customer to proceed to Cashier for payment of installation cost

  • DURATION:
    • 15 minutes

  • OFFICER IN CHARGE:
    • Inspector

  • FORM:
    • Service Application Construction Order (SACO)
    • Inspection Slip

  • END OF TRANSACTION

  • STEP 2

  • CLIENT :
    • Pay necessary cost of materials, fees and charges indicated in SACO

  • SERVICE PROVIDER:
    • Accept Payment & issue official receipt
    • Forward all documents to Commercial Department for preparation & approval maintenance order & action slip.

  • DURATION:
    • 10 minutes

  • OFFICER IN CHARGE:
    • Cashier

  • FEES & CHARGES:
    • Application Fee: ₱ 150.00
    • Service Fee:
    – Tabing Linya – ₱ 200.00
    – Bagtas – ₱ 300.00
    – Boring – ₱ 500.00
    – Materials: ₱ 2368.25
    Note:
    • Cost of Materials varies depending on the location & size of mainline concrete demolition & restoration
    • Fees, Charges and cost of materials are subject to increase without prior notice in case of inflation

    • Sign Action Report Form upon completion of installation

  • SERVICE PROVIDER:
    • Installation of Water Service Connection
    – Tapping
    – Boring

  • DURATION:
    • 5 working days (Tapping)
    • 10 working days (Boring)

    • Maintenance /Plumber

  • FORM:
    • Action Report Form
    • Acknowledgement Slip
